Ork leaders are just darker than everyone else.
Canonically they're bigger. There's a whole bit about it in Da Uvver Book (one of the books from Gorkamorka - it established much of the modern Ork lore).
Thought it was just whoever smashes in the most heads. You might be bigger but if tiny over there smashes your head in, he's in charge of you now
