You forgot the part where the anesthesiologist is out of network, but you were not told this nor were you offered any other options before your treatment, and now your bill has doubled.

The in/out-of-network / copay / waiver / out-of-pocket limit debacle is a whole different bass-ackwards fuster-cluck...
