I'm not sure if it's a visual thing or if it actually didn't give me any medals :D

That is what shows when you have 250 Medals already, since it can't give you any more. If you have 243 it will show 7 medals as the reward, since that would top you off.

Are you already maxed out at 250 medals?

Yes! Was saving them for a new warbond I got later. Good to know.
