Been drinking exclusively (give or take) no-sugar caffeinated sodas since a child. Not obese or diabetic. Mid-thirties now. How bad is this?

Exactly this. It is the reason I got gout.

Yes it is also a genetic predisposition thing but the moment you find out it's to late.
