Regional accessibility would be my guess. The service isn't available in that person's country/location.

Oh its complex but a Healthcare system built to profit off the sick and a work culture that emphasizes stress and competition and directly and indirectly punishes wellness are surely a part of the problem. Cheap sugar too.

Probably haven't gotten regulatory approval in that particular country. Some features in smartwatches are considered as medical devices.

My Fitbit for example will stop reading SpO2 and ECG outside of countries that have approved it.

Money, and not caring about maintaining infrastructure or lives (or if caring, not able to do either competently, or those who would are not in charge), probably.
