Her expression is shy instead of assertive. Her facial features are softer. Skin is smoother. She looks like she's wearing makeup. This one doesn't have as severe an undercut on the right side. The eyebrow grooming is high fem.

Karlach in-game reads toppy lesbian fantasy to me, this reads subby straight girl fantasy to me.

They're both hot AF obviously.
