You're describing what Oracle Intelligent Advisor (OIA) tries to deliver with their interviews. After you write your code, an interview can be automatically generated which collects data and infers which questions need to be asked based on previous answers. For data representation, it is unfortunately either captured in Microsoft Word or within their interview screens as logic, which does not meet your graphical representation needs. It seemed to be close enough that I thought it worth mentioning. It's original use case was for tax purposes but I use it professionally to determine eligibility to public assistance programs.

Similar to @steph's answer ErgoAI is a prolog derivative with a syntax rather similar to .dot (from what I gather). This tool would capture the data in a format you like, but does not generate a fillable form for you, but I believe it could be made to do so in a manner similar to OIA above.

Here's the tldr of the backstory for those as clueless as myself:

Colleen Ballinger, known for her character Miranda Sings, is facing multiple accusations of inappropriate relationships with minors and bullying. Ballinger created Miranda Sings as a parody of teenage girls aspiring for fame on YouTube. However, former fans are now expressing their annoyance and disappointment with her. The first allegations against Ballinger surfaced in a video by a YouTuber named Adam McIntyre in April 2020. McIntyre claimed that Ballinger had interacted inappropriately with him when he was a minor. Ballinger addressed this claim in an apology video. More allegations have since emerged, including accusations of sending lingerie to a fan and engaging in inappropriate conversations in a Twitter group chat with minors. Ballinger has apologized for her past racist and fatphobic behavior and abuse of her childhood dog. The allegations against her resurfaced in 2023 when another YouTuber named Kodee Tyler Dahl shared their experiences and screenshots from the Twitter group chat. The chat revealed conversations where Ballinger asked inappropriate questions to underage fans. Ballinger has not directly commented on these specific allegations.
