Or even if that's the main reason the person went to therapy, perhaps they weren't looking for a "how do I turn this frown upside down" solution, instead hoping to get some advice on how to better channel their frustrations, or gain clearer focus, or just to have someone to help sort out own thoughts. Bad: "It's not my fault", "I was upset", "I didn't do anything wrong", "my tummy hurt", etc and so on.

Good: "I'm sorry, when I did X it [upset/disappointed/hurt/etc] you. I shouldn't have [punched/licked/posted/etc] the thing. Going forward I will be careful to [de-escalate/social distance/double check/etc]." and then do as you said.
