I think part of it is what you listed but I also think part of it is to review your update for its architecture or your decision making. For example we had a system which read serial numbers with a barcode scanner. For some reason the original designers decided the user had to scan the sn off the paperwork then off the device then the system does another automated scan off the device. The software verified the first two were identical but accepted anything for the third. After launch we discovered a bug where sometimes the third scan would report the scanners status instead of the serial number. Another developer was assigned to fix the bug. His solution was to filter out anything from the scanner that starts off "status:" this would pass all tests but it's poor design. In the review it was brought up that it should be verified against the first two, not just that it's not a status message.

That's the real value of code review. Not "defects" but inefficient solutions which technically work for the tests.

Fun fact due to this and weird political incidents a tortoise was the only creature to survive the Crimean war. Basically the war declaration included a town in Britain which is a part of Britain but not in Britain. Said town was not included in the peace treaty, as a result technically the war went on for over 200 years between Russia and this town even though fighting stopped way earlier. A tortoise was on one of the ships at the start and was still alive when this town signed it own peace treaty with Russia. Since it went on for 200+ years, no human survived the entire thing making this tortoise the only known survivor.

In my last company we used a system called windchill. Technically they had folders. Previously we used a different system. But when we switched to windchill no one had time to actually sort and organize the tens of thousands of documents. As a result everything just got dropped in the root folder.

To make it worse there was no enforced naming scheme... Plan for... Thing's plan.... Protocol for execution of thing... Ip of thing.... Thing's up... Protocol of thing.... Plan of thing... All valid. And in 5 years when your 3rd replacement is trying to find it... Alcoholism is a serious disorder

Exactly, it's not for everyone but if you like it you really like it. You obviously like it and if compared to other stuff it's a great value. Watch a movie at the theater... Spend 100s of hours in factorio, both are probably close to same price and the theater possibly much more expensive after drinks and snacks
