If you usually wear belts and you have back pain maybe you could start from that. Better the annoyance of your pants not staying in place than the back pain.

That's certainly worse. But to be more clear what I meant is that tight belts are more of a problem than they might seem.

Do tight belts cause back pain? I’ve never heard that. Also I’ve had some back issues over the years, and no doctor or therapist has ever told me that.

Mainly they tell me to exercise, and give you special exercises to do. Those help.
