Bugger!

A spokesperson for the Gillies Group said all proper procedures were followed following the incident and no one was hurt.

I'm gonna hazard a guess that someone put more weight on the back hoist than it was supposed to carry, or moved the arm out further than it was supposed to go with that weight.

Definitely, my guess would be whatever they were lifting was heavier than they thought.

These trucks have a chart on them that says how much you can lift at a given extension of the crane, and if you exceed that, over she goes.
